At your first family medicine visit, we will take time to get to know you and discuss your past medical, surgical and family history, as well as review your current medications and allergies. See below for information specific to our individual clinics.

What is Family Medicine?

A family medicine clinic can see and treat patients of all ages. While pediatricians care for children and internal medicine physicians care for adults, family practitioners roll these practices into one specialty. Because we care for members of all ages, many families come to see us and develop lifelong relationships. Visiting a family doctor allows you to foster healthy lifestyles in your children; since they come to see us regularly as they grow up, they become comfortable with our office and have confidence that we will be familiar with their evolving medical needs. Becoming familiar with a patient through every stage of their life allows us to diagnose and treat issues quickly, make more informed decisions, and direct you to necessary specialists as needed.

Care Management Program

Who are our Care Managers?

Our Care Managers are professionally licensed nurses, who understand the health care system, its complexities and the services within the community. They are part of your personalized plan of care, working with your Primary Care Physician to access high quality and cost-effective options to meet your health care needs.

How Can Care Managers Help Me?

Upon contact, our care managers will schedule a visit to meet with you in order to conduct a thorough assessment of your health care needs. They will identify barriers, the need for assistance and/or services, review medical history and your current health status. Together, you develop a personalized plan of care to help you achieve and maintain your health goals. They work with your provider as part of your health care team to ensure you get the best possible care.

Services Offered:

  • Health assessments and identification of needs
  • Care planning and recommendations
  • Coordination of medical and community resources
  • Screen, arrange and monitor services
  • Counseling, education and support
  • Provide information and assist with the selection of a retirement community, assisted living facility and/or skilled nursing facility
  • Advocacy and monitoring to assure quality of services
  • Serve as a liaison with health care providers
  • Communicate with families and care providers

TeleHealth

To help alleviate the need to leave the house during the state-wide Stay-Home requirement, we are now offering TeleHealth services with our Primary Care Providers at Scheurer Health. Patients can arrange for a virtual family medicine appointment with their provider utilizing their own personal smartphone with just some easy and simple steps.

A TeleHealth Overview

Call your provider's office and ask for a virtual appointment through TeleHealth

Our clinic team will ask whether you would like the secure link sent to your phone or email address. You should receive the link almost immediately.

When your appointment time is approaching, follow the link and check-in. In order for your provider to see you, click the blue "Enable Camera" button and then "allow" to access your camera and microphone. You are now in the virtual waiting room.

When your provider is ready to meet, your screen will automatically update and connect for your appointment.

Please note:
TeleHealth services require certain system updates and requirements. All devices must have a functioning camera/webcam. Some functions or tests may not be capable via TeleHealth and may require an in-person appointment to be scheduled afterward.

Policies

  • Your insurance cards and driver's license are required for each visit.
  • Arrive 10 minutes prior to your scheduled appointment time to allow for any computer system updates.
  • If you are more than 15 minutes late, you will be rescheduled for another day.
  • Cancellations: We kindly request that you provide 24 hours' notice.
  • No shows: Three missed appointments could result in discharge from the practice.

Insurance

Our family medicine clinics accept many different types of insurance; please call ahead to make sure we accept your insurance.

Fees

To keep billing costs down, payment is expected at time of service. This includes co-pays and deductibles. We accept cash; checks; and Visa, MasterCard or Discover cards.

Prescriptions

For your safety, prescriptions can only be refilled if your examination is current. When requesting medication refills, please allow 72 hours for telephone and written prescriptions.
