The MyChart will take you to a site that offers numerous healthcare facilities. Choose Scheurer to access Scheurer’s facilities. Contact medical Records to gain access to your MyChart Patient Portal account. If you already have an account through Covenant’s MyChart, you already have access to this service. If you are interested in accessing the portal please contact Medical Records at 989.453.5235.
If you would like to appoint someone else to have the ability to access your medical records through the portal please complete the Adult Proxy form. Please submit the form to Medical Records. Keep a copy for your records.
If you would like to access your child’s MyChart, click here. Please be aware that children ages 12 to 17 must also sign this form. If your child is under the age of 12 only the parent needs to sign the form. If the child is under the age of 12, access will terminate at the age of 12 and a new form with their signature will need to be submitted. Please submit the form to Medical Records.

Hospital Patient Portal // prior to March 14, 2020
The Thrive Patient Portal is no longer the main medical record for Scheurer Health patients. If you do not have a MyChart account, in need of medical information that took place before March 14, 2020 and it took place at Scheurer Hospital, click the button below. All other medical information can be accessed via MyChart. This is available to patients 18 years of age and older only.
